NOTICE OF PRIVACY
HEALTH INSURANCE PORTABILITY AND ACCOUNTABILITY ACT HIPAA
Federal and state laws prohibit health insurance discrimination against patients who undergo genetic testing. The Federal Health Insurance Portability and Accountability Act of 1996 protects patients in group health insurance plans from discrimination based on genetic information.
We are required to (1) maintain the privacy of your health information as required by law, including more stringent state laws; (2) provide you with notice of our legal duties and privacy practices with respect to your Protected Health Information (PHI), and to abide by the terms of such notice; and (3) notify you following a breach of your PHI that is not secured in accordance with certain security standards.
NOTICE OF PRIVACY PRACTICES
The HIPAA Privacy Rule gives individuals the right to be informed of Ashion Analytic’s privacy practices and their privacy rights with regard to their personal health information. We reserve the right to change the terms of this Notice and to make the provisions of the new Notice effective for all PHI that we maintain. If we change the terms of this Notice, the revised Notice of Privacy Practices will be made available upon request and or can be downloaded in PDF format here.
